Kristy Neeley, LPN, is a chronic care manager on Pinehurst Medical Clinic’s chronic care and transitional care management team. A familiar face to Pinehurst Medical Clinic, Kristy previously worked on the dermatology team before transitioning to chronic care management. As a chronic care manager, Kristy makes her patients a priority by providing high-quality, patient-centered care, and treating each as family. She enjoys having the opportunity to create meaningful relationships, knowing that she is making a difference in their lives.
