Biography
Hannah McLean, RN, is a transitional care coordinator on Pinehurst Medical Clinic’s Chronic and Transitional Care Management team. As a transitional care coordinator, Hannah works closely alongside patients recently discharged from the hospital. She helps patients by educating them on how to manage chronic conditions and providing resources in their communities to help prevent furth hospitalization.
With a background in emergency medicine, Hannah enjoys how she can build close relationships with her patients in transitional care management and considers it one of her favorite parts of her job. She is her patients' biggest advocate to ensure they receive the best quality of care treating them as family.
When not caring for patients, Hannah is spending time with her family. As a mom of two boys and one girl, most of her free time is spent at the baseball field.
