At Pinehurst Medical Clinic, we take a comprehensive, team-based approach to wellness—recognizing that health is influenced by many factors, including genetics, lifestyle, and emotional well-being. Our Wellness Department is dedicated to supporting each patient’s unique needs through personalized care, education, and ongoing guidance.

Our team of experienced health coaches and specialists come from a variety of educational and professional backgrounds, allowing us to offer a well-rounded approach to improving health and quality of life. Whether you are working toward weight loss, managing a chronic condition, improving nutrition, or seeking support for emotional health, we are here to partner with you every step of the way.

We offer a wide range of evidence-based wellness services, including behavioral health counseling to support emotional and mental well-being, and Diabetes Self-Management Education and Support (DSMES) to help patients better understand and manage their condition. Our Intensive Behavioral Therapy (IBT) for weight loss program provides structured, personalized strategies to help patients achieve and maintain a healthier weight, while Medical Nutrition Therapy (MNT) offers tailored nutrition guidance for a variety of health conditions.

In addition, we provide tobacco cessation counseling to support patients in quitting smoking and improving overall health. For those living with chronic conditions, our specialized programs offer targeted support—helping patients navigate life with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) through education, monitoring, and lifestyle guidance.

Our Services

Behavioral Health Counseling

Feelings of sadness, feeling blue, down in the dumps or worried, nervous, uptight and anxious are all normal feelings. When these feelings begin to take over your daily thoughts and affect your everyday behaviors, it is time to call for help. Your emotional health is just as important as your physical health and both influence each other greatly. The Behavioral Health Counseling program provides a safe, confidential and non-judgmental environment while being guided through your feelings to work toward solutions. This service is provided by one of our Licensed Clinical Social Workers (LCSW). The initial visit will be approximately an hour long and includes a general emotional assessment. You and the LCSW will determine the duration, frequency and length of any subsequent visits. 

Our counselors are in-network with Medicare, many Medicare Advantage, and commercial plans. Our wellness department will assist you in confirming your insurance coverage prior to your first visit.

Diabetes Self-Management Education and Support

Living with diabetes can be very manageable when you have the correct information and the confidence to make healthy decisions that will help control your blood sugar (glucose) levels. With our DiabetesSmart program, we can help coach and empower you through the decision making process. You have access to our diabetes team of dietitians, nurses and health educators who work directly with your doctor. Our DiabetesSmart program consists of an initial one-on-one assessment, followed by group education. During our time together, you will learn more about diabetes and how it affects your body, how to make lifestyle changes to better manage the condition, how to read food labels and determine what a carbohydrate really is and how to fit them into your diet without going overboard. Our goal is for you to feel confident in your ability to live happily and successfully with diabetes. Most insurance plans cover diabetes self-management education and training with a co-insurance. If you have a secondary insurance, it often covers that portion.

The American Diabetes Association recognizes Pinehurst Medical Clinic as meeting the national standards for diabetes self-management education. This is a yearly process that our DSMES team goes through which includes site visits, updating goals and audits. Heart Health Care

PMC is committed to helping patients protect and improve their heart health through early identification and prevention-focused care. Patients diagnosed as being at risk for developing heart failure may have conditions such as high blood pressure, high blood sugar, insulin resistance, obesity, high cholesterol, sleep apnea, medication-related heart risks, or a family history of heart disease. At this stage, patients do not have structural heart disease or symptoms of heart failure, but they do have an increased risk of developing future heart complications. With proactive care, lifestyle changes, and ongoing support from the PMC care team, this stage of heart disease is often preventable and reversible, helping patients reduce their risk and support long-term heart health.

Living in the Heart Zone

Navigating life after a Congestive Heart Failure (CHF) diagnosis can be stressful, but it doesn't have to be. Living in the Heart Zone is a program to help you navigate life with CHF. A health educator, pharmacist, social worker, registered dietitian, and nurse will guide you through the program by providing education, support, and coaching along the way. They will teach you how to recognize the signs and symptoms of CHF, when to call your doctor. how important your diet is, exercise options, and more in your free workbook. Your care team is committed to ensuring you have the proper tools and knowledge to feel as comfortable as possible. 

Medical Nutrition Therapy (MNT)

Medical Nutrition Therapy (MNT) is a service that allows you to meet one-on-one with a registered dietitian to learn about how specific types of food affect health conditions you may have. Some health conditions, such as diabetes, kidney disease, celiac disease, along with others, are significantly improved when changes are made in our daily food choices. In order to make these lifestyle changes, it helps to have a better understanding of food, while also learning strategies and tips to make food selection simpler. MNT can help with this. Medicare and Medicare Advantage plans cover this 100% for those with diabetes and kidney disease. Private insurances may cover this for other diagnoses as well. We can you help determine what your insurance will provide.

One Breath at a Time

Pinehurst Medical Clinic Wellness Department offers a unique program for patients diagnosed with chronic obstructive pulmonary disease (COPD). The program provides education, coaching, and support from a respiratory therapist, clinic pharmacist, registered dietitian, licensed clinical social worker, and registered nurse. Together the team works with the patient to ensure they have the knowledge and resources to live a happy, healthy life.

Advanced Care Planning

Planning for future healthcare decisions can bring peace of mind for you and your loved ones. Our Advanced Care Planning program, led by a Licensed Clinical Social Worker (LCSW), provides compassionate guidance and support as you navigate important healthcare decisions and documentation. We assist individuals and families with understanding and completing Healthcare Power of Attorney forms, Living Wills, Do Not Resuscitate (DNR) orders, MOST (Medical Orders for Scope of Treatment) forms, and Five Wishes documents. Our goal is to ensure your healthcare preferences are clearly communicated and legally documented. For added convenience, notary services are also available to help complete the process with confidence and ease.

April Health

PMC is committed to supporting the mental and emotional well-being of our patients through comprehensive behavioral health services. In addition to PMC’s existing behavioral health counseling offerings, our partnership with April Health, a Wysa company specializing in collaborative behavioral health care, expands access to additional mental health resources and support. This integrated care model connects primary care providers with behavioral health care managers and psychiatric consultation services to help patients receive timely, coordinated care for concerns such as anxiety, depression, chronic stress, and mental health challenges related to physical health conditions. Together, these services strengthen PMC’s commitment to providing whole-person care and ensuring patients have access to the support and resources they need.    

Palliative Care: Support During Serious Illness

For people navigating a serious illness, our referrals to specialized palliative care teams offer an extra layer of support focused on improving quality of life. Unlike hospice, palliative care can be introduced at any stage of an illness and can be received alongside curative treatments. Working in addition to your primary care team, the palliative team helps manage pain and symptoms, reduces individual and caregiver stress, and ensures your personal goals and wishes are carried out.

Hospice Care: Comfort and Peace of Mind

For people navigating the advanced stages of a terminal illness, our referrals to trusted hospice care teams offer a compassionate approach focused entirely on comfort and dignity. Unlike palliative care, hospice is introduced when the focus shifts from curing an illness to maximizing peace of mind and quality of life. Working in addition to our primary care group, the hospice team manages pain and symptoms, provides emotional and spiritual guidance, and offers dedicated support to both the individual and their caregivers during life's most sensitive moments.
