• All gift cards expire 1 year from the purchase date.
• Minimum purchase starts at $20.
• Gift cards must be purchased specifically for a provider or products. You can purchase gift cards for:

• Juliann Wallner, MD – self-pay services only
• Ashley Cochran, FNP-C – self-pay services only
• Jill Johnson, LPN, Aesthetics Nurse- all services
• Amber Peele, RN, Aesthetics Nurse- all services

• Gift cards cannot be transferred to another provider after purchase unless the provider is no longer employed with PMC.
• Gift cards must be presented to the clinic in order to be redeemed. If the gift card is lost, it will no longer be valid.
• Gift Card purchases are final and non-refundable.
